Now Hiring: LVN for Adult Hospice and Home Health

We are seeking a skilled LVN to deliver high-quality nursing care in patients’ homes, under the supervision of a Registered Nurse. This role involves collaborating with the healthcare team, patients, and families to ensure the best outcomes.
Job Title: LVN – Adult Hospice and Home Health
Locations: Lower Valley & Upper Valley
Job Types: Part-Time, PRN
Shifts: Day Shift, Evening Shift, Night Shift, and Weekends as needed
Work Schedule: Monday to Friday, Weekends, Evenings as needed
Key Responsibilities:
Patient Care: Provide direct nursing care to patients in their homes, ensuring the implementation of care plans in line with physicians' orders.
Assessment & Documentation: Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records.
Medication Management: Monitor patients’ medication usage and report results, ensuring proper administration and dosage.
Collaboration: Work closely with the interdisciplinary team, patients, and their families to assess, implement, and evaluate care plans.
Personal Care: Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs.
Care Coordination: Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.
Working Conditions:
Work in patient homes, often in varying environmental conditions (including possible exposure to bodily fluids and infectious diseases).
Ability to travel locally to patient homes and manage flexible work hours.
Occasional emergency PRN calls and exposure to adverse weather.

Qualifications:
Education: Graduate of an accredited school of vocational nursing
License/Certifications:
Current state LVN License.
BLS Certification (required).
Valid Driver’s License.
Experience: Minimum of 2 years as an LVN in Clinical, Hospice, or Home Health settings (preferred).
