OPPORTUNITY Tri-Cities Community Health Center is looking for a compassionate, patient oriented, and engaged pharmacist to join our Clinical and Pharmacy teams! This position offers a very diverse role including, assisting with prescription refill authorizations for our providers and providing clinical support to our providers and patients in the form of consults and chronic medication reviews, annual wellness visits and hospital follow ups with occasional staffing in our retail pharmacy settings.
Essential Functions - Perform patient visits to manage chronic disease states per referral to from primary care providers working under a CDTA.
- Assures that the clinic maintains standards of pharmacy practice equal to or above those accepted by the profession and the law.
- Performs the duties of a pharmacist, including dispensing prescriptions, and counseling patients on their medications.
- Maintains accurate documentation in the pharmacy software system
- Clinical and Technical:
- Complies with clinic policy and procedures consistently in all aspects of employment.
- Participates in meetings as directed.
- Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment
- Promotes effective working relations and works effectively as part of a department/unit team inter and intra departmentally to facilitate the department's/unit's ability to meet its goals and objectives
- Perform other duties as assigned
Shift: Full Time 8-hour shifts, 8AM to 5PM Monday through Friday. No weekends or major holidays.
Location: Based in our Kennewick Auburn Street Location
Requirements Education: Bachelor’s Degree in Pharmacy or Pharm D. from an accredited pharmacy program.
Experience: Experience in clinical pharmacy and/or residency in ambulatory preferred.
License/Certification: Current Washington State Pharmacist license in good standing.
Knowledge/Skills/Abilities: Working knowledge of pharmacy processing software, inventory and cost management activities. Clinically competent. Strong problem-solving, communication and computer skills required. Knowledgeable regarding current standards of practice and competencies of Pharmacists as endorsed by the WA State Department of Health.
