Community Health Workers (CHW) at Neighborhood have an important role in achieving an excellent patient experience. We're looking for a CHW who is kind and professional, listens to patients and seeks solutions.
About The Role As a CHW you will assist patients and families with accessing, navigating and connecting with internal providers and services, outside community resources and programs. Responsibilities include:
- Assists patients in accessing community resources upon provider referral to, but not limited to, food pantries, clothing, shelter, transportation, education, parenting or other services as needed
- Assists patients in accessing health care by assisting in the completion of Medicaid, Child and Family Health Plus and Sliding Fee Scale applications. Connects patients to appropriate facilitated enrollers
- Assists patients in accessing and completing referrals as ordered by providers. Obtains prior authorizations as needed. Documents and tracks referrals until completion
- Conducts community outreach to identify people who are experiencing homelessness and educate them about health needs
- Supports people who are experiencing homelessness in accessing health care services at the Health Center
- Serves as health and community resources coordinator for patients and providers
- Consults with providers regarding patient needs and making appropriate referrals or linkages
- Advocates for patients when interacting with community agencies and services
- Conducts outreach and speaks to community groups
- Assists in developing Health Center staff in improving patient navigation, decreasing barriers for patients in accessing care and connecting patients to care as appropriate
You will be primarily based at our Blasdell location, 4233 Lake Ave., Blasdell, NY 14219.

Education And Skills To Be a CHW - High School diploma
- Ability to travel for community outreach. Available for extended hours once a week 7-9 p.m.
- Experience in Community Health, Patient Resource Coordination or equivalent experience
- Medical office and medical terminology knowledge and experience preferred
- Exceptional attention to detail and follow-through
- Strong skills in utilizing Electronic Health Records (EHRs), Microsoft Office Suite, and other computer software packages
- Superb organizational skills
- Strong influencing skills
- Self-starter with the ability to work proactively and take initiative
- Results-oriented; creative and persistent in accomplishing tasks
- Solutions-driven with a systems orientation
- Highly responsive, timely, accurate and thorough in follow-up and follow-through

Must be available to work Monday-Friday between 8:30 a.m. and 4:30 p.m.
What We Offer Compensation: $22.25 an hour based on a full time, 40 hour workweek
