Job Description

Overall Summary:

The Community Health Worker II (CHW II) will primarily serve and assist patients across various sites and community events. CHW II will work in different capacities based on their assigned area of training and specialty. As a trusted member of the community, the CHW II will act as a liaison between primary care, specialties, and available resources. Activities may include outreach, engagement, care coordination, case management, and linking patients to resources such as food, housing, financial and transportation assistance, as well as care within the clinic.

Essential Responsibilities:

Under the general direction of the supervisor/manager, the Community Health Worker performs a variety of services. Duties include:

  • All CHW I tasks, plus:
  • Completion of CEC certification to determine eligibility and enroll patients in health plans
  • Analyze screening tool results and share with the Care Team
  • Link patients to services within the clinic and the broader community
  • Participate in staff meetings, case conferences, and training
  • Collect data and document in client files as required by agency and funders
  • Support deliverables for various grants, including eligibility assessments, socioeconomic needs assessments, and addressing medical disparities
  • Assist families with crisis intervention, advocacy, problem-solving, and other appropriate interventions
  • Use de-escalation techniques when appropriate
  • Utilize program evaluation tools (e.g., pre/post-tests, satisfaction surveys), and report on outcomes
  • Provide resources on health topics such as nutrition, physical activity, hygiene, sexual health, and substance abuse
  • Maintain accurate and confidential records of interactions with patients, families, and stakeholders
  • Work flexible hours, including evenings and weekends
  • Work at various locations including clinics, mobile units, schools, homeless encampments, warming centers, patient homes, or appointments as assigned

Secondary Responsibilities:

  • Attend workshops, training, and meetings as needed or requested
  • Perform other duties as assigned by supervisor

Qualifications:

Required Education, Experience, and Training:

  • High school diploma or equivalent
  • 1–2 years of experience in a community-based healthcare service capacity preferred
  • Bilingual preferred
  • CHW Certification (with recertification and/or retraining as required)
  • CEC Certification

  • Health navigation and care coordination

  • Familiarity with common treatment protocols

Work Conditions:

  • Travel to other sites and/or locations required
  • May visit patients in their homes or accompany them to appointments
  • May require working outdoors in varying temperatures
  • Prolonged periods of standing or sitting

Physical Requirements:

  • Sitting, walking, and standing typical of an office environment
  • Regular bending, stretching, and lifting
  • Manual dexterity for standard office equipment and computer keyboard
  • Exposure to outdoor elements during travel
  • Presenting information to individuals or groups
  • Reading and writing for record accuracy
