Role Description
This is a full-time on-site role for a Community Health Worker at Secured Health. The Community Health Worker will be responsible for conducting health assessments, providing resources and referrals, advocating for patients, and coordinating care plans. This role will be primarily based in Baltimore, MD, but will also be in other areas of Maryland.
Qualifications
Strong communication and interpersonal skills
Reliable Transportation
Experience in conducting health assessments and care coordination
Knowledge of community resources and social services
Ability to empathize and advocate for patients
Fluency in languages spoken within the community is a plus
Ability to work independently and collaboratively
High school diploma or equivalent (training or certification is a plus)
