Independent Hospital system seeks a Family Medicine Physician to work in the outpatient setting near Warren, Pennsylvania! (30 Minutes from Jamestown, NY and 3h from Cleveland, OH)

This is a Non Profit group located along the beautiful Alleghany River in Pennsylvania! Easy access to Buffalo or Pittsburgh!

• 8 Total Locations in the greater Warren, PA area
• Located in the area since 1900 and serving around 40,000 residents

Providers Role with the Company: Family Medicine Physician

• Location: Warren, PA
• Outpatient primary care in the clinic setting
• Monday - Friday 8am to 4:30pm
• Call:1 Week every 2 months
• Estimated Patient Volume: 18-25 Per Day
• PA and NP support staff
• Assess, maintain, and improve the health status of individuals in the communities served through timely access to evidence-based acute, preventive, and chronic disease care.
• Serves as a role model of professionalism to all members of the interdisciplinary health care team. Collaborates with all other practitioners and clinical support staff to form a high performing team.
• Provides high-value care; demonstrates good stewardship of resources by limiting unnecessary testing and referrals; and considers risks, benefits, and patient preferences when making diagnostic or therapeutic intervention decisions.
• Documents all patient care activities and services provided in the electronic health record. Documentation must be clear, concise, accurate, updated, and in sufficient detail to allow another provider to continue the patients care as well as to allow other parties to review the safety and quality of care provided.
• Internal Medicine Candidates: Cardiac management care is a plus. Ability to manage long term medication, read stress tests, EKG, and PFT is beneficial. Additional bonuses provided for these services.

Background Profile: Family Medicine Physician

• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Ability to hold an unrestricted state of Pennsylvania MD or DO licensure
