This position is for a Home Health Physical Therapist responsible for providing physical therapy services to patients in their homes within East Portland, OR, and surrounding areas. The role involves patient care, documentation, and collaboration with a team to optimize patient outcomes. The therapist is expected to have their own transportation and manage a caseload with productivity standards.

Responsibilities:
- Provide physical therapy services to patients in their homes.
- Conduct patient assessments and develop treatment plans.
- Document patient progress and maintain accurate records using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis.
- Collaborate with the healthcare team to coordinate patient care.
- Manage a caseload and meet productivity standards (30 points).
- Travel to patient homes within a service area, with occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
- Perform Oasis documentation.
Required Skills & Certifications:
- Active Physical Therapist license.
- BCLS certification.
- 1 year of skilled home health experience.
- Oasis experience.
- Must have own transportation and car insurance.

Special Considerations:
- The facility can call off UP TO one shift per pay period (every two weeks).
- Traveler is expected to have their own transportation.
- Business casual attire, no jeans, scrubs allowed but not required (no specific color).
- Considered local within 50-75 mile radius, will accept locals at the same rate.
- First-timers accepted.
- Pending License Accepted.
- RTO Restrictions: Confirm with hiring manager.
